CPR certification and OPIQ permit for private practice
Mandatory attestation upon registration of respiratory therapists

I, the undersigned, a respiratory therapist registered on the QuickConnex platform, declare and confirm the following:
- I hold a valid and up-to-date cardiopulmonary resuscitation (CPR) certification, in accordance with the requirements applicable to my professional activities.
- I hold a valid permit to practise from the Ordre professionnel des inhalothérapeutes du Québec (OPIQ) as well as the authorization required to practise in private practice.
- I undertake to keep my CPR certification, my OPIQ permit and my private-practice authorization valid and up to date for the entire duration of my professional activities carried out through QuickConnex.
- I acknowledge that I am fully responsible for maintaining, renewing and ensuring the validity of these professional documents and authorizations.
- I undertake to immediately inform QuickConnex of any expiry, suspension, restriction or change affecting my right to practise, my private-practice authorization or my CPR certification.
- I understand that QuickConnex may ask me at any time for proof of a valid CPR certification, OPIQ permit or private-practice authorization.
- I acknowledge that any false declaration or failure to meet these commitments may result in the suspension or deactivation of my access to transfer requests on the platform.
This declaration is stored electronically by QuickConnex as proof of the respiratory therapist’s professional commitment.
